Though we have been asking to fill out the questionnaire and consent form everyone to visit our clinic each time, we will change the regulation and the form from August 3rd.

*Fill out the form before visiting clinic and bring it.
*If you check YES, you may not enter clinic. Please call us for changing your appointment.
*We ask all the patients bring the completed form.
*If you would not bring the form, you may need to fill out it outside of clinic. (The form is at the entrance.)
We are working to effort to prevent from infecting Covid-19.
